HOW
DO I....
OBTAIN
A REPEAT PRESCRIPTION?
Patients on regular
medication will require repeat prescriptions and, for a safer and speedier service,
repeat prescriptions are now computerised. Your doctor will have entered onto
the computer what medication you are taking, and each time a new prescription
is written by the computer a new request form is also generated for you to make
your next request.
Requests for repeat prescriptions are not taken over the telephone, for medico-legal
reasons, as errors can occur in verbal communication.

If you have more than one repeat
prescription please try to order all your items together. |

Please enclose a stamped addressed envelope with your
request if you wish us to post it back to you. If you are a Lewes patient and require your prescription to go to
St Annes, Boots or Bakers, please indicate this on your prescription request. Patients at Ringmer can request
theirs to go to the Lloyds Pharmacy at Ringmer by indicating this on their request slip. |

Either deliver or post your
prescription request form to the surgery. |

If you are receiving any medication,
which is not on your request form, please add this clearly on the request slip and this will be added after
verification with the doctor. |

Please always enclose your computer
counterfoil, which carries your code number, with all repeat prescription requests. |
Prescriptions will be ready for collection 48 hours, ie two working days after being
requested.
Prescription Charges
People in the following
categories are automatically exempt from prescription charges:
Children under 16; those under 19 who are in full-time education; people over
60 years of age; pregnant women and women who have had a baby in the last 12 months;
people in receipt of DSS benefit and those with specific medical conditions.
Pre-payment Certificates
It is usually worthwhile
to buy a pre-payment certificate if you have several
items. We have application forms available at the practice.
HOW DO I....
OBTAIN A SICKNESS CERTIFICATE?
You do not require
a doctor's sickness certificate for any illness lasting seven days or less.
Your employer may, however, require you to complete a self-certification form
(SC1), which is available from your employer. For any illness lasting longer than
seven days you will need to see the doctor for them to issue a sickness certificate
(FMed3) and for any subsequent removal of the certificate.
HOW DO I....
SPEAK TO MY DOCTOR?
You may telephone your GP during the day and, if they are available, they will talk to you between consultations. However, this may not be convenient and you may be asked to phone back at the end of surgery. Alternatively, your GP will return your call if you have been unable to speak to them.
